OASIS HOUSE-Outpatient and Day Rehabilitation Services SCHARP provides case management, mental health services, medication services and day rehabilitation services to mentally ill adults living in the community.

AB2034- provides needed case management, mental health and medication services to homeless metally ill individuals in the community. The goal of the program is to deliver integated, cost effective services that reduce homelessness, substance abuse, recidivism in incarcerations and psychiatric hospitalizations, and increase employment.

Dual Diagnosis- The program provides case management mental health services to mentally ill persons with a co-existing substance abuse disorder. SCHARP provides these servies to clients residing in Department of Mental Health service area six and eight. The services are designed to provide ongoing linkages needed resoures, mental health counseling and development of functional independent living skills.

Cal Works- The Department of Public Social Services (DPSS) funded program services are designed to reduce mental health barriers that interfere with welfare recipients ability to return to work. Services provided are outreach, case management, medication services and mental health counseling and living skills development.

Step Out Apartments- Provides transitional housing services in 10 two bedroom townhouse style apartments in the unincorporated area of Willowbrook for Transitional Age Youth (18-21). Supportive services are provided to assist youth in increasing independent living skills in preparation for moving to permanent housing upon completion of the Transitional program.

LOS ANGELES Homeless Services Authority Programs (LAHSA)

Milestones- (SHP 2000) Provides outreach and supportive mental health services to individuals who are homeless, mentally ill, substance abusers. The goal of this program and all HUD/LAHSA sponsored programs is to obtain permanent housing for the designated population.

Homeless Outreach Services- (SHP 2000) Provides outreach and supportive mental health services to individuals who are homeless, with co-existing mental health and substance abuse disorders. The service goal is to obtain and maintain permanent housing.

Latino Outreach Services Program- (SHP 2000) Provides outreach and supportive services mental health services to latino individuals who are homeless, mentally ill, substances abusers with a goal of obtaining and maintaining permanent housing.

Oasis House Supportive Services- (SHP 2001) Provides outreach and supportive mental health services that result in obtaining permanent housing to African American and latino individuals who are homeless and mentally ill. 65% of the population will have a co-existing substance abuse disorder.
